The Shield of Andrem
This item appears to be an old, if well made +1 shield. However it has a number of abilities locked within it that wielders can gain, it's surface changing as it evolves.
The first level is unlocked when the wielder, while using the shield, is struck by a critical attack. From there on the levels can be unlocked as you, the DM, see fit, such as; leveling along with the player, when the character meets certain goals, a great RP moment, or even a certain quest is completed ("you must carry the shield to the Pool of Many Tears during the eve of the ninth full moon and bathe it in the water therein." etc.etc.).
It's entirely up to your style of play (heck it could even be sentient, with it's awareness increasing with each level).
Level 1 - When struck by a critical attack it rings like a churchbell and can be heard in a 30' radius. All allies who are able to hear it can choose to spend 1 hit die to regain health.
Level 2 - The shield can now be used as an off-hand ranged weapon that deals 2d4+STR bludgeoning damage before returning to your arm. The range and strength of the churchbell increases to 60' and 2 hit die.
Level 3 - As a reaction the shield can be thrown between an ally (or other) and an incoming attack to add it's AC to their own. The range and strength of the churchbell increases to 120' and 3 hit die. Shield Throw can now target three enemies in sight dealing 1d6+STR bludgeoning damage (separate attack rolls for each target, the shield immediately returns on a failed attack)
Level 4 -The shield fully awakens and grants you sovereignty over the lands of Andrem.** The people will rally behind you, even against the wishes of their leaders. Churchbell now reaches 240' and 6 hit die. Shield Throw now targets up to 9 enemies in sight and deals 2d8+STR bludgeoning damage. (Note: the idea behind the massive stat increases of this level is that the character would be fighting on a battlefield amongst many allies and enemies.)
