This is just an idea I came up with earlier today that I thought I would post for anyone who wants to use it, or offer feedback on it. I'm calling it Shadowreach Prison, and it's inspired by the Eyrie's Sky Cells from A Song of Ice and Fire/Game of Thrones.
Basically, the idea is that there is this prison that's underground, probably far underground. Each cell has two doors, one of which locks from the outside and leads into the rest of the prison. The other door presents the true horror of this prison. It locks and opens from inside the cell, but instead of leading into the prison or back to the surface, it opens into a tunnel that connects to the Underdark. So prisoners inside these cells are faced constantly with the possibility of freedom, if only they can survive the horrors of the Underdark. Not to mention the fact that they would have to do so unarmed and unarmoured, at least at first.
I just thought this was a cool concept that could lead to some interesting stories or adventures, depending on how you wanted to implement it.

Super cool idea. What's lurking just on the other side of the doors? Maybe you defeat it and can arm yourself with the things that are dead there. Maybe staging a group uprising, or a crazy awesome place to start a campaign. Really like this one because like you said it could lead to a ton of interesting stories and adventures.

I love it! It would be the ultimate prison break game (...campaign?). I would love to play a game where you are stuck in the underdark with nothing but your wits. It would be short lived, but really fun.

Oh I like the idea. You could be really demented, and let it be like the Eyrie, but underground. Say it's built in a giant Stalagmite (from top down if I am wrong) and it's in a large cavern. One side is the door that led in, and the other side is an opening to pitch black cavern. What PC's don't know, is at the bottom is a large lake, which technically makes the fall survivable. So when a captive finally decides the darkness is better then waiting, and step off the edge to end it all, they hit the water, and are swept off into the Underdark, which wasn't the quick death easy exit they were hoping for. Fun stuff though, your idea is cool, I like it.

This is awesome! I think I'm going to build this into my world. Even if this group of PCs are never imprisoned there I like the idea of having it in for flavor at the least. Maybe they'll eventually run into the guy that escaped? Could build an entire legend around the one man/woman that took that route and got out. Is it true? Did the owner of the prison start the legend to give false hope?

What if the door to under dark opens only once a week and only for a specified time. Kinda like the maze runner you can explore only so far before running back. And with the door open I deranged dark creatures can enter. It would be terrifying to have the door opened and your locked in your cell, or the craziness and pressure to run through it on limited time
Maybe prisoners can opt to run it, and are given a random bag of items like in battle royal. But the wizard warden skryes them to watch like a show such as running man.

This concept is top-notch on the awesomeness scale! I couldn't possibly write out all of the possible ideas. I think the prison doesn't have to be limited to the Underdark either. There could be doors to the elemental plane of fire, the shadowfell,and all sorts of wacky stuff! I love the utility of it.

Yeah, this Idea ties into a setting I already had in mind really elegantly. I like the stalactite idea. When I first read it I imagined it built around a central lava tube. levels upon levels of doors honeycombed around this central shaft. Never really being able to each other, occasionally hearing rusty hinges creak then a whoosh into the darkness, perhaps a scream that starts just as it begins to fade into the depths. The sound of large claws skittering along the cavern walls just outside the cell door. Roar of something large from the depths. great setting. love it.
