Reddit user u/Neskatomon made some great combat tracker worksheets last year, (which is available here) www.reddit.com/r/DnD/comments/2d0y02/5th_edition_combat_tracker/And I've modified them to suit my own needs. Grab them off my google drive if you've a mind to. drive.google.com/folderview?id=0B1o6wUkfnuRDWWJkNHlZLUg0YmM&usp=sharingI happened upon these multicolored plastic stands for them which are available to buy here, blankdice.co.uk/Board_game_accessories/23/Ive been matching them with d20's in order to help keep track of my monsters when im running games, and modified the sheets to match as well. I like crossing off boxes rather than rewriting numbers got them laminated and have been using wet erase pens and they've worked great! have a variety of HP levels so far, and will probably do something with 700 or so at some point when my players get ready to take down Tiamat or the terrasque or someone. Ill be adding more as the need arises. also took away the initiative tracker on the 5e Player tracker (the original of which is available here,) dngnsndrgns.blogspot.com/2014/10/fillable-d-5e-encounter-crib-sheet.html and added a flaws and bonds section, since i don't really need yet another initiative tracker, and i'm always forgetting to use them anyways. Hope someone finds this useful! If anyone has an improvement or suggestion, feel free to have at it.

Photo doesn't seem to want to load for some reason. But here's how the system is supposed to work.
