They did indeed first show up in the Fiend Folio, and we're apparently borrowed/inspired by something George R. R. Martin wrote. The Spelljammer setting had the illithis and gith rivalry prominent, and introduced Gith Pirates as the big bad to watch out for. Their ships were awesome looking. They are another of the races that has made it into every edition so far. I don't read much of George Martin, but I'm pretty sure they come from his sci-fi novel, Dying of the Light (which explains why the name was familiar). So are these creatures essentially in space? Yes, but in a goofy D&D context. The Spelljammer cosmology looks like this: Each world (campaign setting) exists in its own space/time bubble. These bubbles float in a sort of interstellar goo called Phlogistan (spelling?). The concept of Spelljammer is that some folks build magic ships that can sail through this substance. So you could take a ship and visit the Forgotten Realms, kill some Drow, hop back aboard and voyage over to Eberron to sack some ancient giant ruins and recruit a Warforged bodyguard. From there you can head on over to Ravenloft and have the Warforged act as your front line tank against the vampires. You get the idea.
