I constantly say yes to my players and now they are stupid OP... Opps. 1 of my fighters routinely deals 100+ damage per round and one shot a CR 13 dragon at level 10 my other fighter routinely deals 50+ damage per round.
At level 11 I sent them to the abyss to "speak" to the Prince of hell. (Not telling them he was a Balor Demon) They had originally planned to kill the Prince. (DM Note: The Balor Demon had a staff that banishes things to the plain of elemental fire. Simple, Players go in cocky, attack the Prince, he laughs and sends them to the elemental plain of fire.)
Well, the Kitsune charges, blenders her way into the prince, doing obscene amounts of damage. (Not enough). The orc Fighter charges in next, coming down with her axe and taking this CR 20 Demon down to 10 HP... (Wow)
No problem, it is over, the demon stands up laughing maniacally, raises his staff and... the player says "oh, i forgot to add the demon bane"
I sigh. the prince dies. and they get to level up because he was worth that much XP. then I realize I just gave them a stick that banishes things they hit to the plain of elemental fire. ugh

Saying yes isn't bad, but with certain requests it can be better to use 'yes, but'.
And if it's a magic item overload, don't forget to enforce the attunement rules.
But everyone has campaigns that fly away from us, we just have to learn from our mistakes and continue being the insane rulers of universes we know we are.

I will admit. It has led to some hilarious games... My Bard dominated a succubus.
